CM Care Record Update

The CM Care Record will be updated in early 2026 (date to be confirmed). 

This update, called Version 4 (pictured below), introduces a more user-friendly interface with improvements to navigation, accessibility and overall performance, providing a smoother and more intuitive experience. 

All existing information will remain available. The refreshed design will make it quicker and easier to access the information needed to deliver timely, informed and coordinated care.
