Accessing the CM Care Record

The CM Care Record is accessed through a system called CareCentric, provided by Graphnet. 

If your organisation can view the CM Care Record (see the Data Visibility Guide), you will access it in one of the following ways: 

  • Either directly through your existing clinical Electronic Patient Record (EPR) or care system using Single Sign On (SSO), or 

  • Via the Graphnet Shared Care Record Web Portal. A username and password will be supplied to you. Access is requested through your IT Service Desk. ​

IMPORTANT PLEASE NOTE - In some clinical and care systems, the CM Care Record may be listed by a different name. This could include either the CM Care Record, Shared Care Record, Graphnet Shared Record, Graphnet Portal, CareCentric, CareCentric Portal, Cheshire Care Record (CCR), or St Helens Care Record.

Requesting access 

If your organisation is able to view the CM Care Record (see the Data Visibility Guide) but you are unable to access it through your existing clinical or care system, please contact your IT Service Desk for support or get in touch. 

Role-Based Access Control (RBAC) 

Access to the CM Care Record is governed by Role-Based Access Control (RBAC), as defined by each participating organisation. This means that your level of access within the CM Care Record reflects the information you are authorised to view within your own organisation. 

This approach ensures that professionals can only access information that is relevant to their role, supporting safe, effective and secure use of the record. All access to the CM Care Record is fully logged and audited.
