Guidance for professionals

Across Cheshire and Merseyside, work is underway to deliver a single, region-wide shared care record platform called the Cheshire and Merseyside Care Record (CM Care Record).

The CM Care Record enables authorised health and care professionals to securely view relevant information from other organisations across the region, supporting timely, informed and coordinated care.

The CM Care Record Programme Team is working with local health and social care partners to introduce additional clinical information into this shared care record, ensuring you have access to the information you need at the point of care.

To see which health and care organisations are currently sharing and viewing information through the CM Care Record, please refer to the Data Visibility Guide.

If your organisation can view information through the CM Care Record, the sections below explain how to access and use the system. You can also find further information about the planned update to the CM Care Record, due to go live in early 2026. This update will introduce a more user-friendly interface, with improved navigation, enhanced accessibility and better overall performance.
