Access

The CM Care Record is accessed through a system called CareCentric, provided by Graphnet. 

If your organisation can view the CM Care Record (see the Data Visibility Guide), you will access it in one of the following ways: 

  • Either directly through your existing clinical or care system using Single Sign On (SSO), or 

  • Via the Graphnet Shared Care Record Web Portal. A username and password will be supplied to you. Access is requested through your IT Service Desk.

Technical and data issues 

If you are a health or care professional and experience any technical issues when accessing or using the CM Care Record, or if you notice any problems with the information displayed, please contact your organisation’s IT Service Desk in the usual way. They will advise on next steps and escalate the issue where appropriate.
