Information included in the CM Care Record

The CM Care Record brings together information from a range of health and care services, including: 

  • GP (primary care) services 

  • Hospitals (secondary care) including specialist services such as The Clatterbridge Cancer Centre and The Christie Cancer Centre 

  • Community services 

  • Mental health services 

  • Social care services 

It will include information for people who are registered with a GP in Cheshire and Merseyside, in addition to those who have received care from participating health and social care organisations in the Cheshire and Merseyside area.

The type of information that may be viewable includes: 

  • Basic patient demographic details - for example your age, address and NHS number 

  • Medications, treatments and allergies 

  • Diagnoses and medical conditions 

  • Test results - such as blood tests, scans and X-ray reports 

  • Your care history and activity - including appointments, hospital stays, A&E visits, referrals, discharge summaries and clinical letters 

  • Care plans and notes - such as treatment plans, case notes, alerts, waiting list status, and any preferences or decisions about your care 

  • People involved in your care - including care teams, keyworkers and relevant services. 

Participating organisations and data visibility

Some health and care organisations in Cheshire and Merseyside already share and view information through the CM Care Record, and both data sharing and visibility are continuing to expand.

How your information is used 

Your information is only used to support your direct care. It is not used for research or planning. 

By providing a single, secure view of your health and care details, the CM Care Record will help ensure that the professionals involved in your care have the relevant information they need when they need it. This allows teams to coordinate more effectively, ensuring your care is timely, appropriate and consistent with your needs. 

How your information is kept safe and secure 

All information accessed through the CM Care Record is handled in line with strict data protection and Information Governance (IG) standards. 

Only authorised health and social care professionals involved in your direct care can view information relevant to you. Each organisation controls access based on a person’s professional role, so staff can only view the information they are authorised to see. 

Access to the CM Care Record is read-only, meaning professionals can view information but cannot change it. Every access is securely logged and audited to ensure your information is used appropriately, safely and only to support your direct care.
