How the CM Care Record will work
Right now, each health and social care organisation keeps its own records about the people it supports.
The CM Care Record will bring this information together to create a single, shared view of your health and care information across Cheshire and Merseyside.
It will enable authorised professionals – such as your GP, hospital team, community nurse, mental health worker or social care team – to see the information they need to support your care, all in one place.
By safely joining up information about you, the CM Care Record will make it easier for professionals to work together and avoid the need for you to repeat your story.
Most importantly, it will help improve lives by supporting safer, more joined-up and more efficient care. It means professionals can see the right information, at the right time, helping them make quicker, better-informed decisions about your care.
Our Data Visibility Guide outlines what information is currently shared by participating organisations in Cheshire and Merseyside and who can view this information. This guide continues to be updates as access to the CM Care Record is expanded across Cheshire and Merseyside.
