About the CM Care Record

The CM Care Record (CMCR) - formerly known as the Cheshire and Merseyside Connected Care Record - is helping to improve lives by digitally enabling joined-up health and social care across Cheshire and Merseyside. 

Delivered by NHS Cheshire and Merseyside on behalf of the Cheshire and Merseyside Integrated Care System (ICS), the CM Care Record will enable health and care professionals in Cheshire and Merseyside to view relevant information for the people they support – no matter which organisation recorded it. This joined-up approach will help ensure that everyone involved in your care has the information they need, when they need it, to make timely and informed decisions about your health and wellbeing. 

For example, a hospital doctor will be able to see key details from your GP record – such as diagnoses, medications, and recent test results. They will also be able to view relevant social care information, including care packages in place, details of carers or support workers, and any recent changes to home care arrangements. 

This broader view removes the need for you to repeat the same information at every appointment and helps professionals provide safer, more coordinated and more timely care. 

The CM Care Record is also a key part of how health and care services in Cheshire and Merseyside are working together to deliver integrated care and meet the requirements of the NHS 10 Year Health Plan for England - making care more seamless, more proactive in supporting early intervention, and more centred around people’s needs.
